Iron Infusion Program · California

Start your iron infusion evaluation

A short screening, a $99 cash-pay lab panel at Quest, then a physician consultation with Dr. Misra. No insurance and no referral needed.

How it works

How it works

Four steps from sign-up to your consultation. Each payment is made before its step and is non-refundable.

  1. 01

    Step 1

    Screening and sign-up

Answer eight short questions and give us the details for your chart and lab order. It takes about five minutes.

  2. 02

    Step 2

    $99 lab panel at Quest

Pay the $99 lab fee to our office and we email your requisition. Visit any Quest location in California with only your photo ID and the requisition. Quest bills our practice directly, so you do not pay at the lab. Do not give Quest your insurance information: that may trigger a separate, much higher bill outside this program. No fasting needed.

  3. 03

    Step 3

    Medical intake

Once your lab results are available, complete your medical intake form.

  4. 04

    Step 4

    Physician consultation

Once your labs and intake are complete, you pay for and schedule a 30-minute consultation with Dr. Misra. The consultation is a separate $350 for self-referrals and outside physician referrals. If iron is ordered, treatment is paid for before it is ordered and scheduled.

1. Where you live

2. Screening questions

A short screening, not a full medical history. Dr. Misra collects the detailed history in your intake and consultation.

Are you currently pregnant, think you may be pregnant, or breastfeeding?

Are you currently receiving treatment for an active cancer?

Do you have severe kidney disease or are you currently receiving dialysis?

Do you have heart failure or another significant cardiac condition?

Have you ever had an organ transplant?

Are you currently under the care of a hematologist, oncologist, nephrologist, cardiologist, or transplant specialist for a condition that may affect iron treatment?

Have you ever been diagnosed with a hemoglobin disorder such as thalassemia, sickle cell disease or trait, hemoglobin C, hemoglobin E, or another inherited blood disorder?

Have you ever had a serious allergic reaction to IV iron?

3. Your details

Used to create your chart and your Quest lab requisition. Enter your name as it appears on your photo ID.

4. Before you continue

The $99 lab price depends on these three points. Please read and tick each one.

Your information is kept private and used only to coordinate your care. See our privacy policy.

You pay the $99 lab fee on the next screen. Our office then emails your Quest requisition and medical intake.
