This form is a crucial part of your application and must be completed by a Civil Surgeon designated by the Department of Homeland Security (DHS).
What is a Civil Surgeon?
A Civil Surgeon is a licensed medical doctor who has been authorized by the U.S. Citizenship and Immigration Services (USCIS) to conduct immigration medical examinations. These physicians have met specific requirements and have demonstrated their ability to perform the necessary examinations and complete Form I-693 accurately.
Why is Form I-693 important?
Form I-693 is essential because it ensures that individuals seeking to adjust their status to permanent residence do not have any medical conditions that could pose a public health risk or make them inadmissible to the United States. The form also verifies that the applicant has received all required vaccinations.
What does the medical examination involve?
During the medical examination, the Civil Surgeon will:
Review your medical history
Perform a physical examination
Conduct a mental health evaluation
Screen for tuberculosis, syphilis, and other communicable diseases
Verify that you have received all required vaccinations
Once the Civil Surgeon has completed Form I-693, they will seal it in an envelope and give it to you. Do not open this envelope; instead, submit it unopened with your adjustment of status application. The sealed Form I-693 is valid for two years from the date of the Civil Surgeon's signature
